CareCycle Management | Navigator®

The CareCycle Navigator is a 2015 SXSW Interactive Innovation Awards finalist. This responsive web application allows for remote clinicians to effectively and efficiently monitor high-risk patients. The system receives real-time vital data and prioritizes patients based on their daily risk levels. The platform utilizes historical, demographic, and vital data to ensure each patient is monitored in the correct context and evaluated thoroughly. Patients monitored through this coordinated care solution, have a re-hospitalization rate of 4.5%, compared to the 28% national average of this patient demographic.
